Application for NC Health Choice for Children.

Pregnant women, parents, or other adult relatives who live with and care for qualifying children may also use this application to apply for Medicaid for themselves. It is a two-page application form which may be submitted to your County Social Services department by mail or dropped off at the DSS office.

Aged, Blind or Disabled Medicaid Application

This applicationis intended for medical assistance for the aged (over 65), blind, disabled, or those who want Family Planning services (who are not a caretaker of a child under age 19).  You will need to list all persons who need medical assistance.
